Subsidy Rental Assistance Program
Depending upon the availability of funds, MEO’s Community Services Department will process applications according to the following set of priorities. Priority listing of applications shall be based on the applicant’s ability to meet the highest number of the following criteria (as solely determined by this agency):
RENTAL CHECKLIST
Applicant must meet one of the categories:
For County (Rental Assistance Program)
Target Population:
- Homeless
- At-risk of Homelessness
- Emergency or transitional housing
- 80% of median family income
For State (Hawaii Public Housing Authority)
Target Population:
- TANF eligible person and/or families but not receiving and/or
- In possession of Section 8 Voucher (Award Letter from HUD) or
- Homeless families transitioning to permanent housing.
Categorical eligibility:
- Applicant has one (1) minor child under 18 yrs. old living in the house (birth certificate needed) or pregnant woman in the last trimester if there are no other children 18 yrs. and younger. (Physician’s Statement as to status of pregnancy is needed)
- Gross monthly income is < 185% of Federal Poverty Level
- Available resources/assets is < $ 5,000.00
For both programs:
Applicant must have the following:
- PHOTO IDENTIFICATION of applicant and all adults 18 years +
- Social Security Number/Birth Date of all household members
INCOME VERIFICATION: (Applicant must provide all source of family income applicable)
- Current Pay Stubs (2) from all household members covering 30 days prior to application date
- Social Security Statement of Benefit, pension
- Monthly/quarterly financial statement; and GET if self-employed
- DHS Notice of Approval for Financial/FS Benefits
- Supplemental Social Security Income
- Other (child support, alimony etc.)
ASSET VERIFICATION
- Current Checking Account Statement
- Current Savings Account Statement
- Other
LEASE VERIFICATION
- Copy of signed lease agreement (1 year lease only) or
- Unexecuted (unsigned) lease agreement (1 year lease from date of application) if not currently living in the rental unit at the time of your rental application. However, check will not be released to landlord until submittal of signed lease/rental agreement.
- Landlord’s General Excise Tax Number (GET)
- Tax Map Key of the rental unit
- W-9 Form with Landlord’s signature
EVICTION VERIFICATION
- Eviction Letter addressed to the applicant or
- Letter from Public Housing, Homeless or transitional shelter addressed to applicant
Eviction letter must include the following:
- Date of Eviction Letter
- Date applicant must vacate the premises
- Breakdown of costs and total amount owing
- Printed name and Signature of evicting landlord
- Contact number of evicting landlord
OTHER: (As needed only)
- Receipt of payment of back rent
- Rental Subsidy Letter (HUD or DHS)
- Payment arrangement (with dates of scheduled payments) signed and dated by both applicant and landlord
NOTE: PROGRAM IS DEPENDENT UPON AVAILABILITY OF FUNDS; AND FOR THE SUBSIDY RENTAL ASSISTANCE, NO ASSISTANCE IS GIVEN FOR BACK RENT, FIRST MONTH’S RENT OR SECURITY DEPOSIT.
